Actually, I agree with the second statement. These incidents (taking off with less runway margin than supped by the performance calculations) happen much more often than they should. Either due to taking off from the wrong point, for having entered the wrong weight, for having the airplane misconfigured (flaps), or other reasons.

part of this problem is aviation's absolute OBSESSION with acronyms and short hand. for fuck's sake! the entire world manages to work just fine without acronyms and shorthand for EVERYTHING. Aviation is so screwed up they even use the same acronyms to mean two different things, as in the error in waypoints made by some crew in south america once upon a time...
